Visit & Free Consultation
Schedule a free consultation with a caring member of the Constellations: Grow and Play team. We’ll connect by phone or a virtual meeting to learn more about your child, answer your questions, and explore whether our in-home support is the right fit for your family. This step is all about helping you feel informed, comfortable, and confident moving forward.
Enrollment Form
Once we’ve talked through how we can support your child, the next step is completing our Enrollment Form. This helps us gather the details we need to thoughtfully tailor services to your child and ensure a smooth, supported start to your journey with us.
Assessment
Your child will participate in a comprehensive assessment led by one of our Board Certified Behavior Analysts (BCBA). This helps us understand your child’s strengths, support needs, and learning style so we can create a thoughtful, individualized plan that supports meaningful progress and growth.
